A premier Veteran's Health Administration facility providing specialty care to our nations heroes.
In order to become a youth volunteer, you must meet the following qualifications:
• Be 14 Years Old or older
• Enrolled in High School
• Have a 3.0 or Higher GPA
• Must be a US Citizen
• Must be willing to commit to 80 hours, minimum
• Have written permission from a parent or guardian
• Be willing to obtain TB testing
• Will need two academic references
• Attend orientation
• Comply with all the guidelines outlined on page 8 of this application
The steps to becoming a volunteer:
1. Fill out the attached application and return to Voluntary Service for review, include an official copy of your Transcripts.

2. Submit to a TB test (PPD or Quantiferon Gold) with your private doctor.
If you are positive for TB, please obtain a Chest X-Ray and present that to Occupational Health in lieu of a positive TB test.
The TB test will must be approved by Occupational Health in Building 30 (see attached Map).
Occupational Health is open Monday-Friday 7:30am - 3:00pm.
3. Wait for your references to respond to emailed questionnaire, these references must have a .org or .edu email address (contact Ms. Schoenborn if this is a concern).
4. Attend Orientation on the specified date.
Note: if you have a family member working at JAHVH, you will not be assigned to that area.
Assignments for volunteers include: Whole Health, Nutrition, Morning Coffee Service, Assisting Recreational Therapy, Escorts, Logistics, Clerical Positions, Greeters, Kiosk Assistance, My Life, My Story and Transport.
This application is due by March 22, 2019 at 5pm, late submissions are not accepted.
